All athletes need these forms completed to be able to participate for the upcoming school year.
If you registered your child for a previous season, in the current school year then if it says submitted, you should still click to choose your child's new sport in the current season to register them. You do not need to re-complete all the forms if you have already done so for the current academic year.
WJHS/HMS Athletics Physical, Conduct and Concussion Form (Follow the directions below to complete the forms)
Registration for SHIELD Illinois Testing:
-Parents should use a desktop or laptop to navigate and sign in to ps.wilmette39.org
-Choose Forms on the left side
-Select COVID Testing Consent
-Complete the form for each child before the day of testing
For Conduct and Concussion Form:
Use ps.wilmette39.org to
-Sign up for a sport
-Submit an affirmation of parent and student expectations
-Submit a Concussion Education certification
-Submit a current physical
-To pay for sports fees - See information in Fees (Fees will be assessed two business days after a sport is selected. Cut sports fees are due September 2nd.)
